On Friday, 3 June 2022, at Biohacker Summit 2022 Helsinki in Helsinki, Finland, scientist and life science entrepreneur Sakari Nikinmaa delivered a presentation with a deceptively simple title: *Hack Your Oral Health*. What followed was not a standard talk about brushing, flossing or cosmetic dentistry. It was a sharp, unsettling account of how the mouth functions as both a biological frontier and a systemic threat, a place where chronic inflammation can begin quietly and travel widely.
Nikinmaa’s central claim was clear. Oral health dictated systemic health. The mouth, he said, was tied to the gut through the microbiome, to the heart through blood circulation, and to chronic disease through inflammation. In that framework, the teeth were not isolated structures but exposed platforms where disease could organize itself.
“Teeth are the body's only non-shedding surface, making them a special place where bacteria can attach and form chronic biofilm structures that do not naturally renew like the rest of our body,” Nikinmaa said.
Why Teeth Became A Refuge For Persistent Bacteria
The power of Nikinmaa’s talk lay in the way it reframed a familiar problem. Teeth, unlike skin or many internal surfaces, do not renew themselves every few days. That made them uniquely vulnerable. “The blood acts as a taxi service, shuffling oral bacteria all around your body, which is why it is extremely important to prevent chronic inflammation of the teeth to protect your heart and systemic health,” he said.
This was not framed as a speculative link. Nikinmaa pointed to associations between dental disease and more than 200 chronic conditions, including diabetes, cardiovascular disease, cerebrovascular disease, chronic pulmonary disease, dementia, psoriasis and lung cancer. Some of those links remain more firmly established than others, but his broader point was difficult to ignore: chronic oral inflammation was not a niche issue. It was entangled with the biology of long-term disease.
In diabetes, he noted, the relationship was bidirectional. Periodontal disease could worsen systemic inflammation and increase insulin resistance, while poor diabetes control could, in turn, intensify periodontal damage. The mouth and the metabolic system fed each other’s decline.
Why Brushing Often Was Not Enough
If the science sounded daunting, the practical implication was worse. Even diligent brushing, Nikinmaa argued, often failed to fully remove oral biofilms.
He illustrated the mismatch in scale with memorable force. Bacteria existed in the micrometre range. Toothbrush bristles were vastly larger. The surface of enamel, meanwhile, was not smooth but pitted and irregular, full of microscopic niches where plaque could remain after cleaning.
“Removing microscopic bacteria with a toothbrush is like trying to remove sand from the road with a rake; you cannot remove it all, leaving residual biofilm where dental diseases can begin,” he said.
That image captured the quiet frustration at the heart of oral care. People could do many things right and still leave behind the biological scaffolding of disease. Mechanical cleaning remained essential, but in chronic cases it was not always sufficient.
