Siim Land’s Case For A Longer Life That Stays Healthier To The End
Modern medicine had already delivered one of humanity’s quiet revolutions by helping push average life expectancy far beyond what earlier centuries could imagine. Yet at Biohacker Summit 2023 Amsterdam, held in Amsterdam, Netherlands, on 14 and 15 October 2023, the Estonian author and high-performance coach Siim Land argued that the real crisis of aging had not been how long people lived, but how long they stayed well.
In his presentation, *The Longevity Leap: Achieving Biological Youth & Bridging the Gap Between Healthspan & Lifespan*, Land set out a stark arithmetic of modern survival. Global life expectancy had risen to 73.2 years, he noted, but healthy life expectancy lagged behind at just 64. That left a gap of more than nine years, a long corridor of decline in which millions spent the closing chapter of life contending with diabetes, heart disease, stroke, disability or neurodegeneration.
Land’s central proposition was both simple and urgent: longevity was not merely about adding years, but about compressing morbidity, pushing disease ever later so that frailty occupied months rather than decades.
The Nine-Year Gap That Framed The Debate
For Land, this was the fault line in contemporary thinking about aging. Public conversation often celebrated lifespan while neglecting healthspan, as if the extension of life alone were enough. But he insisted that a longer life, if surrendered to chronic illness, was an incomplete victory.
“The goal to extend your healthspan is to ensure you delay chronic diseases for as long as possible,” he said. “Getting a chronic disease early in life shortens your life expectancy much more than getting that same disease later in life.”
The implications were profound. A diagnosis of diabetes in one’s thirties or forties could strip away more than a decade of life expectancy. The same condition, diagnosed in the seventies, carried a much smaller penalty. In Land’s framing, the battle against aging was therefore not only about prevention, but timing. Delay mattered. Every year a person remained free of chronic disease was not just a healthy year gained, but a buffer against premature mortality.
What Centenarians Seemed To Understand
To make the point, Land turned to centenarians, not as mystical outliers but as biological clues. People who lived beyond 100, he said, rarely escaped disease by miracle. More often, they postponed it.
Researchers had divided them into three broad groups: survivors, who developed chronic illness before 80 and still reached 100; delayers, who postponed illness until after 80; and escapers, who reached 100 with little or no major chronic disease at all. The striking finding, Land noted, was that roughly 87% of centenarian men and 83% of women fell into the latter two groups.
That distinction mattered. These were not merely people who lived longer while carrying illness. They were people whose biology, lifestyle or environment had helped defer the common diseases of aging until the very end.
Land invoked the case of Jiroemon Kimura, widely recognised as the longest-living man on record, who reached 116 and remained largely disease-free until very late life. Kimura developed kidney disease only at 113 and experienced repeated heart failure in the final six months of his life. Morbid though it sounded, Land suggested this represented the ideal compression of morbidity: a long stretch of health, followed by a brief period of terminal decline.
Biological Age, Not Calendar Age
At the heart of the talk was a distinction that has become central to longevity science: chronological age is not the same as biological age. Two people born in the same year can inhabit very different bodies by the time they reach 60, 70 or 80.
“You can control the speed of your biological aging and determine how fast you age,” Land said. “Everyone has a different biological age, and it is not always the same as your chronological age.”
His case rested less on futurism than on familiar markers of health. Youthfulness, in this account, was not cosmetic. It was measurable in muscle mass, cardiorespiratory fitness, metabolic markers, cognitive sharpness and resting heart rate. A younger biological age meant a body whose systems still behaved like those of someone far younger, resisting the slow accumulation of dysfunction that made disease more likely.
Fitness As A Shield Against Early Decline
Land’s most forceful argument came when he discussed physical fitness, particularly VO2 max, the standard measure of cardiorespiratory capacity. He described it as one of the most powerful predictors of survival, with low fitness carrying a mortality burden so severe that, on current evidence, it could be worse than smoking.
“Maintaining cardiorespiratory fitness as you age significantly increases your longevity and reduces your risk of mortality,” he said. “Based on the research, not having cardiorespiratory fitness is actually worse for your health than smoking.”
It was the kind of statement designed to jolt a room to attention, but Land used it to underline a broader truth: the body’s decline is not fixed at the same rate for everyone. Exercise could slow it materially. Older adults who trained consistently could retain functional capacity decades beyond their sedentary peers. An 80-year-old who exercised regularly, he noted, could display the strength and aerobic function of a sedentary 50-year-old.
He paired this with another often overlooked marker: resting heart rate. As people age, it tends to rise. Yet a lower resting heart rate remained associated with better cardiovascular fitness and lower mortality risk. In the data Land cited, a resting heart rate of 80 or above carried a 45% higher mortality risk compared with 45 beats per minute.
Heart rate variability, another sign of cardiovascular resilience, also entered his account. It generally declined with age, but daily movement helped preserve it, with benefits appearing to rise steadily even up to 17,000 steps a day.
Why Strength Mattered More Than Size
If aerobic fitness was one pillar of healthy aging, muscle strength was another. Land was careful to distinguish strength from appearance. Low muscle mass was harmful, he said, but low strength was more dangerous still.
“Low muscle mass is detrimental for longevity, but low muscle strength is even more critical,” he said. “Strength prevents falls and preserves physical function, which are essential for staying healthy and avoiding the biggest killers of the elderly.”
This was not rhetoric. Frailty, in older age, often announced itself through loss of function before loss of life. Falls, fractures and the cascade that followed could be catastrophic. The inability to catch oneself, recover balance or rise from the ground often marked the beginning of severe dependency.
Land also pointed to a growing link between physical strength and brain health. Those with the lowest strength, he noted, faced markedly higher risk of dementia and dementia-related mortality. The body and brain, in this view, were not separate domains but parts of the same aging system.
Metabolic Health And The Hidden Burden Of Visceral Fat
If one theme ran through the presentation, it was that modern chronic disease often shared the same metabolic roots. Heart disease and neurodegeneration might appear different in the clinic, but beneath them sat overlapping drivers: insulin resistance, inflammation, hypertension, obesity, poor blood sugar control and lipid dysfunction.
Land focused in particular on metabolic syndrome, defined by a cluster of markers including high blood pressure, elevated fasting blood sugar, high triglycerides, low HDL cholesterol and a large waist circumference. At the centre of that cluster was visceral fat, the fat packed around the organs, less visible than subcutaneous fat and far more dangerous.
He offered practical ways to monitor risk. Waist-to-hip ratio, he said, remained one of the simplest screening tools people could use at home. For women, a ratio of 0.86 or higher indicated high risk; for men, 1.0 or above suggested the same. Body mass index, though cruder, still had value at population level, with the lowest mortality risk generally falling between 20 and 25.
The message was not aesthetic but clinical. Preventing visceral fat accumulation was one of the clearest ways to slow the machinery of premature aging.
Sleep, Lifestyle, And The Unromantic Foundations Of Longevity
For all the summit’s biohacking associations, Land’s prescription was strikingly unglamorous. Healthy aging, he suggested, was built less on exotic interventions than on the durable habits public health has long recognised: sleep, movement, diet, weight control, smoking avoidance and social connection.
Sleep, especially, was treated not as an indulgence but as a biological necessity. Less than seven hours, he said, was associated with a higher resting heart rate and worse metabolic health. Seven to eight hours remained the general target for adults.
He also stressed that healthy lifestyle factors retained their power across the life course. It was not too late in midlife or later life to improve outcomes. The cumulative effect of healthy behaviours, he argued, continued to shape life expectancy well into older age.
A Call To Protect The Body And Mind Together
Land ended with a framework rather than a miracle cure. The longevity leap, as he defined it, required preserving three things at once: physical health, cognitive function and youthful biomarkers. None could be neglected without cost. A strong body paired with failing cognition, or good blood work paired with severe frailty, did not amount to biological youth.
“To achieve the longevity leap, you must maintain physical health, cognitive function, and youthful biomarkers,” he said. “By slowing down the speed of your biological aging, you protect yourself against heart disease, cancer, and neurodegeneration.”
That was the deeper argument presented at Biohacker Summit 2023 Amsterdam by Siim Land. Aging, he suggested, was neither wholly fixed nor wholly conquerable. But between resignation and immortality lay a more humane ambition: to live longer, to remain capable, and to push the great diseases of later life as far toward the horizon as possible.
