By the time Dr. Harri Helajärvi asked the audience to rise from their seats at Biohacker Summit 2017 Helsinki, the gesture no longer felt symbolic. It felt urgent.
In his presentation, *Stand Up For Your Health!*, delivered on Friday, 13 October 2017 in Helsinki, Finland, Helajärvi set out a troubling case against one of the most ordinary features of modern life: prolonged sitting. His argument was simple, but deeply unsettling. The body, he said, was not being undone only by what people failed to do in the gym, but by what they did for the rest of the day, hour after hour, in chairs, cars, offices and sofas.
Helajärvi, a physician working in Sports and Exercise Medicine whose research had focused on sedentary behaviour and its health risks, framed the problem as the product of success itself. Urbanisation, technological convenience and economic progress had made life easier, more connected and more efficient. They had also, almost imperceptibly, made it more passive.
A World Built For Stillness
The story Helajärvi told was not one of individual weakness, but of structural change. Cities had drawn people closer to services, jobs and one another. Cars had multiplied. Home technologies had exploded. Screens had colonised work and leisure alike. Labour that once demanded movement had steadily given way to work performed sitting down.
“Physical activity at work is decreasing,” Helajärvi said. “It used to be that we went to work to do some physical stuff, but now we work passively and must find time for exercise because we do not get it naturally anymore.”
That shift, he suggested, lay behind a grim parallel rise in obesity, type 2 diabetes, metabolic syndrome, fatty liver disease, cardiovascular disease and premature death. Progress had not caused these conditions in any simple sense. But it had created the perfect habitat for inactivity.
The Body Keeps The Score
What made Helajärvi’s talk striking was the degree to which he moved beyond the usual advice about exercise. Yes, planned physical activity mattered. But it was no longer enough, he argued, to think of health as a 30-minute appointment with exertion.
The real question was broader and more uncomfortable: what happened during the other 15 or so waking hours?
“You can engage in health-enhancing physical activity, but is it a truly healthy lifestyle?” he asked. “We need an attitude change where we think about what we do during our entire day, not just the few minutes we spend at the gym.”
His research pointed to a sobering conclusion. People who met exercise recommendations could still incur serious metabolic harm if the rest of their day was dominated by prolonged sitting. Sedentary behaviour was not merely the absence of exercise. It was a physiological state with consequences of its own.
Seven Centimetres, One Screen, And A Warning Sign
Helajärvi described findings showing that among people grouped by overall physical activity, those who spent three or more hours a day watching television had waist circumferences roughly seven centimetres greater than those who watched an hour or less. The pattern held even among the physically active.
That detail landed with force because it challenged a comforting myth. A hard workout could improve fitness, but it did not grant immunity from the effects of long stretches of immobility.
The same pattern emerged in studies of liver health. Following participants over a decade, Helajärvi and colleagues observed clear differences in liver markers among those with high levels of sedentary time. Ultrasound examinations showed that people with prolonged sitting habits had more than double the risk of fatty liver, regardless of diet, alcohol use or structured exercise.
This was not, he made clear, a distant threat. Conditions once thought to take decades to develop were appearing far earlier. Fatty liver disease, he said, was increasingly being seen in young people after just 10 to 15 years of sedentary habits.
What Sitting Does At The Genetic Level
The most compelling part of the case came not from epidemiology alone, but from biology.
“High amounts of prolonged sitting are the most harmful sedentary behavior,” Helajärvi said. “The genes that control glucose and fat metabolism are down-regulated when we sit, but they work normally in those who stand up every now and then.”
He pointed to research in which one group sat continuously for two hours after lunch, while another sat for the same period but stood briefly every 20 minutes. Muscle samples revealed a striking difference. In the uninterrupted sitting group, genes involved in glucose metabolism, fat metabolism and inflammatory regulation were suppressed. In the group that stood up intermittently, those genes continued to function normally.
The implication was profound. The body responded not only to exercise sessions, but to interruptions in stillness. Tiny acts of movement, repeated throughout the day, helped preserve metabolic function in ways that a single bout of training might not.
The Importance Of Activity Peaks
Helajärvi spoke of “activity peaks”, the short bursts of movement scattered through an ordinary day. Their significance, he argued, had been underestimated for years because public health messaging focused so heavily on sweat, intensity and formal exercise.
“The more activity peaks you can get, the more important it is,” he said. “It is not just about sweating and exercising; now we know how much light physical activity and changes in position matter for your health.”
One office worker who exercised dutifully each morning still spent the rest of the day almost entirely still. Another, less athletic colleague accumulated frequent small movements throughout the working day. New measurement technologies, Helajärvi said, showed that these micro-movements mattered far more than conventional thinking had assumed.
This was the hidden arithmetic of modern health: a life could look disciplined in the abstract and still be biologically sedentary.
Mortality In The Long View
Then came the longest shadow of all. Helajärvi referred to a 13-year follow-up study indicating that nearly one in five people who spent most of their time sitting had died by the end of the period, while those who were more regularly upright and active had fared significantly better.
The message was not that everyone must stand all day. In fact, Helajärvi explicitly rejected that simplification. Static standing, badly done, could become another form of strain. The answer was movement, not rigidity in a different posture.
Not Just Standing, But Dynamic Standing
At the workplace, Helajärvi urged a practical rethink. Standing desks were not a cure in themselves if they simply turned sitting still into standing still. What mattered was dynamic posture, shifting weight, rotating hips, changing position and interrupting immobility.
“You can stand up more and incorporate movement into your day,” he said. “Your body needs activity so your intervertebral discs get a massage instead of remaining in an immobile position. This is what we should be thinking of at the workplace.”
The prescription was almost modest in its scale. Stand during calls. Rise every 20 minutes. Walk to retrieve what could be fetched by hand. Reduce passive transport where possible. Limit sedentary screen time. Build a day around frequent motion rather than a single burst of redemption.
The False Comfort Of The Gym Alone
One of the most memorable threads in the talk was Helajärvi’s portrait of the modern “healthy” person: driving to work, sitting at an ergonomic desk, driving home, outsourcing effort wherever possible, then claiming wellness through an evening spinning class.
The satire had an edge because it was familiar. A person could train intensely, log their performance, post their achievement and still spend most of life metabolically switched off.
Helajärvi was not dismissing exercise. He was rescuing it from misunderstanding. Fitness mattered. Strength mattered. Cardiovascular training mattered. But none of these erased the biological cost of uninterrupted inactivity.
An Attitude Change, Not A Gadget Fix
What emerged from Helajärvi’s presentation at Biohacker Summit 2017 Helsinki was less a condemnation of chairs than a call for cultural recalibration. Health, he argued, would not be restored by a single device, a standing desk, or a punishing class squeezed into an otherwise motionless schedule. It required an attitude change broad enough to reshape the entire day.
His challenge was unsettling because it stripped away one of modern life’s most flattering illusions: that isolated acts of self-improvement could neutralise a fundamentally sedentary existence.
By the end, when the audience was invited to stand, the instruction carried the force of evidence. Not a wellness cliché. Not a theatrical flourish. A reminder that longevity may depend less on heroic effort than on whether, throughout the day, we simply keep getting up.
